The Georgia Card (“we,” “us”) is a service of Premier Chronic Pain Care, LLC, an Atlanta, Georgia company. We arrange physician certification visits for Georgia’s medical cannabis program. By using this website or purchasing a certification visit, you agree to these terms.
Your payment covers a one-on-one certification evaluation visit with a licensed Georgia physician, including review of your medical history against Georgia’s qualifying conditions and, if you qualify, a signed physician certification, plus guidance on the state’s registry paperwork. Your payment does not purchase a certification outcome, a state card, or any cannabis product. Georgia’s registry fee is charged separately by the state.
Eligibility and certification are determined solely by the physician under Georgia law. Certification is not guaranteed. Statistics on this site describe historical approval rates and are not a promise of any individual outcome.
If the physician does not certify you, your certification fee is refunded in full. Refunds are issued to the original payment method. If you need to cancel or reschedule a visit, contact us and we will make it right; no-show appointments may be rescheduled at our discretion.
You must be 18 or older to purchase a visit for yourself. Parents, guardians, and legal custodians may apply as caregivers on behalf of a patient as Georgia law allows. A valid Georgia ID is required for certification.

Payments are processed by third-party payment processors on secure, hosted payment systems. Card numbers never touch this website. Prices are shown at checkout; promotional pricing may change without notice.
To the fullest extent permitted by law, our liability related to this website or the scheduling service is limited to the amount you paid us. We are not responsible for decisions of the State of Georgia, including registry processing times or card issuance.
